Patterns of life impacting our health confront us in an ecosystem of modifiable social determinants of health.  Our patient-centric approach oversights, engages with and manages this ecosystem. 

The pattern-of-life health ecosystem includes not only modifiable social behaviors, but also family, friends and other caregiver relationships, the utilization of health provider-based and community-based healthcare assets and the relationship between the patient and the healthcare professional.
